The Chest Wall Disorders program is designed to maximize the convenience for patients and families. Care can be received virtual only or in a hybrid model. The following are the most common pathways. Select a pathway, or view all four.

An appointment confirmation will be sent to your email address on file
Log in to the patient portal to enter your insurance information and a credit card on file for billing
If your health plan includes our practice as an in-network provider, we will submit a claim on your behalf. You will be responsible only for your applicable co-pay or deductible as determined by your plan.
If our practice is out of network, payment for the visit is collected prior to your appointment. A detailed superbill (itemized receipt) will be provided after your visit for you to submit to your insurer for potential reimbursement.
Patients who prefer to pay out of pocket may elect self-pay. Payment is collected prior to your appointment, and a 45% discount is applied to the standard consultation fee at the time of payment.

Once your insurer issues an Explanation of Benefits (EOB), the remaining patient responsibility, co-pay or deductible balance, will be charged to the credit card on file.
